I guess you have to go to the Immigration to add (+1 CHILD) in the visa in your passport. There is a special count for that.

You should also contact to your consulate or embassy to legalize the certificate in your country. Probably before the consulate this certificate should be attested in the Ministry of foreign Affairs in Sharjah (Bukheira Corniche, near HSBC).
